anaconda: avoid adding duplicated kernel entries

Browse Source 
List kernel versions without duplicates, even when there are multiple
files related to the same kernel version.
Duplicated kernel versions here caused regenerating initramfs multiple
times and duplicated entries in xen.cfg.
